Use this form to request approval for the following events:
 

• Events sponsored by a CMCP club, team, or group using school facilities beyond the norm.
• Events sponsored by other organizations using any school facilities.

Complete and Submit request at least fourteen (14) days prior to the date of the event.

Janitorial/Cafeteria Services: please submit request at least thirty (30) days prior to the date of the event.

Moderator/Club/Organization will be responsible for payment of service(s).

After request approval, please contact parties involved to ensure everything is in order.
